Position Summary

The role of the Care Coordinator - Licensed Practical Nurse reports to the Practice Transformation Clinical Manager. This position is responsible for the coordination of practice transformation initiatives for patients within primary care locations. This position is responsible for providing direct patient outreach, assisting care managers with care gaps and managing records and data in the EHR and external data portals, and understand quality measures.

Essential Position Responsibilities

  • Provide patients with timely and accurate reminders related to preventative care services
  • Provide pre-visit planning support
  • Request records from external and internal sources and accurately capture data in EPIC
  • Review monthly dashboards and data to participate in prioritizing patient outreach
  • Conduct Emergency Department follow-up calls, providing patients with appropriate and timely resource information and interventions
  • Assist patients in identifying community resources
  • Participate in the analysis of patient data to meet program requirements
  • Assistance with QI projects and program filings
  • Complete additional duties as assigned within the scope of the care coordinator practice
  • Understand requirements for quality measures and HEDIS, to communicate issues in data and documentation

Qualifications
Education: Graduate of an accredited clinical program
Licensure and Certifications: Arkansas clinical license and CPR, required.
Experience: Minimum 3 years' clinical experience required. Attention to detail and exceptional data entry and management are required. Experience with quality measures preferred.

Work Environment: This position will spend 90% of time sitting while performing work in a standard office environment. This position serves a culturally and linguistically diverse patient population. This position has exposure to communicable diseases and bodily fluids.
